Action: The actions are what were taken by you in order to get the work completed. Since you were a part of the project, its success or failure depends to a certain part on your shoulders. Elaborate on the steps taken in order to complete the work in the way it was supposed to be done and the ways adopted to bypass situations. Anything done apart from the regular to make a difference should be included to gain an extra star.
Result: The outcomes of your work are the results. Pick out the positives of the results in order to make a good impression of your aptitude and attitude. Give examples of specific instances to lure the interviewer to believe your stance.
